Medical examinations for skydiving

A medical statement is required to follow a course for skydiving.

Here you can download the form “self-declaration of health and fitness for parachuting (model 2024)”. You may fill out the “self-declaration” yourself. If all questions on this form are answered with “no” you may sign the form. The form is valid for 2 years from the date of signing. Bring the signed form with you on the day of your ground training.

If one or more questions are answered with “yes” you must undergo a physical examination by an examining doctor or authority (due to rules in de medical world your own doctor is not allowed to sign). You can download the form “KNVvL medical certificate sport parachute jumping (model 2024)” here. The medical examiner will determine whether or not you are suitable for skydiving. Upon approval, the signed form is valid for 2 years from signing.

If you have a valid class 1 or 2 medical examination, it will be considered equivalent.
