3Verklaring en ondertekening
Ondergetekende verklaart zich bewust te zijn van het onderstaande:
1. Het beoefenen van of deelname aan het sportparachutespringen is bepaald niet ongevaarlijk en brengt risico's met zich mee.
2. De vereniging en/of Stichting Parachutisten Centrum Teuge en alle met hun samenwerkende instanties en/of personen is niet aansprakelijk voor schade ten gevolge van de voorbereiding voor en/of de uitvoering van het sportparachutespringen, tenzij sprake is van opzet of aantoonbare grove nalatigheid.
3. De ingevulde persoonlijke en/of springtechnische gegevens naar waarheid te hebben ingevuld.
4. Cursisten dienen zich te houden aan de veiligheidsregels geldende bij het Nationaal Parachutisten Centrum Teuge.
5. Ondergetekende, danwel bij minderjarigheid diens wettelijke vertegenwoordiger(s), is (zijn) zich van de inhoud en strekking van deze verklaring bewust en geeft door ondertekening ervan te kennen het daarin gestelde te begrijpen en uitdrukkelijk en zonder voorbehoud te aanvaarden.
6. Bij het volgen van een opleiding word het deel van de opleidingsgeld bestemd voor de sprongen, alleen op basis van medische gronden gerestitueerd. Overige spronggeld dat gestort wordt op een springaccount vervalt na 5 jaar van inactiviteit.
7. Je persoonsgegevens worden conform onze privacy statement bewaard. Ons privacy statement kun je terugvinden op onze website. Bij inactiviteit worden je persoonsgegevens na een periode van 5 jaar verwijderd. Eventuele springsaldi is daardoor niet meer uit te keren.

The undersigned declares to be aware of the following:
1. Practicing or participating in parachuting is by no means harmless and involves risks.
2. The association and/or Stichting Parachutisten Centrum Teuge and all organizations and/or persons collaborating with them are not liable for damage as a result of the preparation for and/or the execution of the sport parachuting, unless there is intent or demonstrable gross negligence .
3. To have completed the personal and/or jumping technical data truthfully.
4. Students must adhere to the safety rules applicable at the National Parachute Center Teuge.
5. The undersigned, or in the case of minors his legal representative(s), is (are) aware of the content and purport of this statement and by signing it declares to understand the provisions contained therein and to accept it expressly and without reservation.
6. When following a course, the part of the training fee intended for the jumps will only be refunded on medical grounds. Other jump money deposited into a jump account will expire after 5 years of inactivity.
7. Your personal data will be stored in accordance with our privacy statement. You can find our privacy statement on our website. In the event of inactivity, your personal data will be deleted after a period of 5 years. Any account balances can therefore no longer be paid out.

4Aanbetaling
Wij vragen om een aanbetaling van € 150,-. Deze betaal je gemakkelijk online.
LETOP: indien je kiest voor de optie via bankoverschrijving dan verwerken wij je aanmelding pas op het moment dat de betaling daadwerkelijk bij ons is bijgeschreven. Zorg dus dat je snel betaalt om je plaats te garanderen! Indien er sprake is van extra transactiekosten (bijv. Paypal) dan zullen wij je vragen dit bij aankomst te voldoen.
Het resterende bedrag dient uiterlijk 10 dagen vóór aanvang van de opleiding te zijn voldaan. De instructies voor de restbetaling ontvang je in de bevestigings e-mail.
Indien je de opleiding minimaal 30 dagen vóór aanvang van de grondopleiding annuleert, dan is een restitutie van € 100,00 mogelijk. Na het verstrijken van dit termijn vervalt restitutie van het aanbetalingsbedrag.
Na het volgen van de grondopleiding of na het maken van één of meerdere sprongen is alleen restitutie van het eventueel overgebleven ‘springsaldo’ op basis van medische indicatie mogelijk. Het verzoek om restitutie moet schriftelijk gebeuren met daarbij een verklaring van je arts dat je om medische redenen binnen de komende twee jaar niet meer kunt springen.

We ask for a deposit of € 150,-. You can easily pay this online. Attention: if you choose the option for payment by bank we will process your registration when the payment has been received. So do not wait to long or you can loose your spot! If there are extra transaction costs (for example Paypal) we will ask you to pay this upon arrival.
The remaining amount must be paid at the latest 10 days before the start of the training. You will receive the instructions for the remaining payment in the confirmation email.

Refund:
If you cancel the course 30 days before following the ground course a refund of € 100,00 can be made. After this term a refund of the down payment expires.
If you cancel the course after following the ground course or after you made some jumps a compensation for the “jump balance” not made can only be done based on medical reasons. The request must be done in writing with attached the declaration from a doctor that explains that you are not fit for skydiving for at least the coming 2 years.

Wanneer je de volledige AFF opleiding volgt met 3 of meer vrienden op dezelfde dag, dan ontvangt iedere deelnemer een korting van € 75,00. De korting wordt verrekend bij betaling van het restbedrag.
When following the complete AFF course on the same day with 3 of more friends each participant will receive a discount of € 75,00. The discount will be deducted when paying the remaining amount for the course.
